Network Health Medicare Diabetes Prevention Program


This program will help you to prevent or delay type 2 diabetes. This program will be facilitated by Network Health lifestyle coaches. This program follows the National Diabetes Prevention Program, curriculum designed by the Centers for Disease Control and Prevention (CDC).

This year long program is designed to help you make small changes that make a big difference to your health. Making lifestyle changes – like eating healthier, being more physically active and managing stress may help you avoid having to manage type 2 diabetes forever.

This class will be held as follows.

  • Weekly sessions for the first 16 weeks
  • Biweekly sessions for weeks 17-20
  • Monthly sessions for weeks 21-26

Please note, we must have 10 registered members to be able to hold a class. You will be notified if we must cancel or reschedule this class. A wellness coach from Network Health will reach out within 5-7 business days of receiving your registration to review your eligibility and class options.

This Medicare Diabetes Prevention Program is supplied by Network Health Ventures and available to eligible Medicare beneficiaries. Medicare beneficiaries do not need to be members of Network Health to participate in the Medicare Diabetes Prevention Program.
