I'm looking for a doctor, facility or clinic
The providers in this search are in-network. To ensure you are choosing an in plan provider when choosing a facility location, please be sure to hover over the map on the left of the screen and review the providers covered for that particular facility. Please note, not all providers at your selected facility may be participating, therefore ensure the provider you are looking for is listed in the map dropdown as you may be responsible for the cost of services provided by facility where a provider is not listed. To find an in-network doctor or facility near you, choose a Network Health plan type from the drop down below. If you receive services from an in-network doctor working at a facility not listed in this search, you may be responsible for the cost of those services. If you're a member and you're not already signed in, go to My Login to sign in to your member portal account for the most accurate results. Our provider network is growing. ProHealth Care is an in-network provider for all Network Health Medicare Advantage Plans as of October 1. This search will be updated in October.

It’s Your Choice
We will provide you with information to help you make informed choices, such as physician’s and health care professionals' credentials. This information, however, is not an endorsement of a physician or health care professional's suitability for your needs.
The providers available through this application may not necessarily reflect the full extent of Network Health contracted providers. There may be providers or certain specialties that are not included in this application that are part of our network. If you don't find the provider you are searching for, you may contact Customer Service at 800-769-3186. We also recommend that, prior to seeing any physician, including any specialists and mental health providers, that you call the physician's office to verify their participation status and availability, since a provider's availability to see new patients may change frequently.
Some network providers may have been added or removed from our network after this directory was updated. We do not guarantee that each provider is still accepting new members.

Out-of-Network Services at Mayo Clinic
Mayo Clinic is not a contracted, in-network provider with Network Health.
While Network Health’s Medicare Advantage PPO plans allow members to see in- and out-of-network providers, physicians and facilities, unfortunately, Mayo Clinic has indicated they will no longer see some Medicare Advantage members.
We want to make sure our members can get the high-quality care they need with providers, physicians and facilities that work collaboratively with us to ensure the best possible health care experience.
For help finding an in-network provider, physician or facility, use our Find a Doctor search or call our member experience team at the number on the back of your ID card.
Clinically Integrated Partner: Health care provider systems, and the individual providers within those systems, who are in a unique collaborative partnership with Network Health to meet the highest quality care standards and assure members are engaged in their health care.
