Evaluating prospective dentists is a diligent evaluation process that we take very seriously. Our goal is to partner with dentists who will deliver the highest value to our members, our employer and broker clients and to our dental network.
Our prospective dentist evaluation process begins when you complete our New Provider Request Form with information about your practice. When we receive your information, we will send written confirmation to the principal business contact you name, and we will start our evaluation process. We will respond to you with a decision within approximately 45 business days.
