Accessing Your Health Information

Network Health uses data to improve your health and health care. We make it easier for you to access your personal health information so you can make more informed decisions about your health care.

Why should I access my health information?

Some health care decisions are easy, like getting a cast if you break your arm. But other situations are more complicated. Does your medical history mean surgery might be risky? Can your caregiver share your complete health history with a new doctor? Having access to your health care information allows you and your provider to work together to ensure you get the services you need, when you need them.

By accessing your health information through a convenient app, you can do the following.

  • Get control over your health information.
  • Make it easy to share your health information with doctors, caregivers or anyone you choose.
  • Get help managing and improving your health through a wide range of apps and other computer-based services.

How do I access my health information?

The process is similar to loading other applications to your device from an app store like the Apple App Store, Microsoft Store, Google Play and others.
  1. Find the app you want to use from this list of applications.
  2. Download the app.
  3. Complete the registration process.
  4. Use the app's menus and prompts to connect the app to your health plan.
  5. Provide your consent to share your health information with the app.

Is my health information private and secure?

As with any interaction over the internet, there is some risk. Network Health takes your privacy and the security of your health information as seriously as you do.

Network Health safeguards your data carefully. However, once your data is shared with an application, Network Health is no longer responsible for the security of that data. This is why it is important to understand the privacy and security policies for any application with which you choose to share your data.

It is also important to protect your devices from threats like theft, phishing and hacking. You should keep your device's security settings current. You can decide whether to access your health information via an app. If you do, make sure you choose a health information app with clear privacy and security policies. You can change your mind at any time.

Why is Network Health allowing apps to connect to my health information?

Network Health’s mission is to create healthy and strong Wisconsin communities and we want to help you manage your own health. This new way to help you access your information is a federal government requirement. The federal government, the health care industry and Network Health are working toward several goals.
  • Patient access - ensuring patients are well-informed about their own health and health care
  • Interoperability - making it easier to share information between patients, doctors, caregivers and other people and organizations working to improve and maintain your health
Some of these goals and rules are covered by a federal law, the 21st Century Cures Act. This is the law that requires and allows Network Health to make your health information easier for you to access.

What is the signup process like?

The first time you choose to share your data with an application, you must set up an account. You will need your full name, date of birth, Network Health member ID number, zip code and a valid, unique email address.

However, once your data is shared with an application, Network Health is no longer responsible for the security of that data. How do I access my health information?

What is the signup process like?
Once you have downloaded and registered for an app you will use the app’s menus and prompts to connect the app to your Network Health data.

  1. You will be prompted to set up an account on the 1upHealth website.
  2. You will enter your full name, date of birth, Network Health member ID number, zip code and your email address.
  3. A code will be sent to your email address.
  4. You will enter the code in the app to confirm that the app can access your data.
  5. Any future apps you want to give access to your data will just require that you enter the email address you used to create your 1upHealth account. A code will again be sent to the email address and you will enter the code in the new app.

Where can I find a list of applications that I can use?
Here is a list of applications that have been evaluated, reviewed and are deemed as qualified and secure applications.

What information do I need to authorize an application to see my Network Health data?
To provide access to your data to an application, you must create an account by providing your first name, last name, date of birth, Network Health member ID number, zip code and unique email address. If you already have an account through our member portal you will use that log in to provide access to an application.

If I have already authorized one application to see my Network Health data, will I have to create another account to authorize a second application?
No. Once you have authorized one application to see your Network Health data, you only have to authenticate using your email address and corresponding code to allow a second application to see the data.

What email is used to authorize an application to see my Network Health data?
You must use the unique email address that you initially used to authorize and allow access to your data. An authorization code will be sent to the email address, and you will be required to enter it into the application. If you do not remember the email address, contact Network Health at the phone number on the back of your member id card to obtain the information.

Are the name fields case sensitive?
No.

How can I find my member ID number?
It is the number listed on the member ID card provided to you by Network Health.

I have authorized successfully, but I don't see any data, or some data is missing?
Contact the application developer via the support mechanism they have provided on the application. The application developer will try to resolve the issue and if they are unable to, they will contact 1upHealth to troubleshoot together until the issue is resolved. The application developer will contact you once resolved.

I am a personal representative for another member (parent of a child, legal power of attorney, etc.). How do I obtain access to the member's data through an app?

I am a member and I want to give permission for my personal representative to access my data, how do I give my permission?

I am a member and I gave permission for a personal representative to access my data, how can I revoke that access?
Contact the Network Health Member Experience team at the number on the back of your member ID card for assistance.

How can I report identity theft and fraud?
If you believe an application you've shared your data with is misusing that information in violation of their stated privacy policy, contact The Federal Trade Commission to investigate the matter by going to ReportFraud.ftc.gov or calling 877-382-4357.

If you believe the privacy of your health care data has been violated, contact the federal Department of Health and Human Services Office of Civil Rights at: hhs.gov/ocr/complaints
