Wellness Programs

As a Network Health Individual and Family Plan member, you have access to the following programs to help you stay well and manage your health conditions. Because we care about your health and wellness, these programs are available at no additional cost to you.

Momentum*

The Momentum wellness program gives you and your covered spouse the opportunity to earn a $100 gift card for completing four tasks essential to your health and wellness.

Two tasks must be completed within 180 days of your plan's effective date and the 2021 calendar year.

1. Designate a personal doctor through the member portal.
2. Complete the Health Risk Assessment (through WebMD®) from the member portal.

Two tasks must be completed sometime during the 2021 calendar year.

1. Receive a flu vaccination and complete the corresponding form found in the member portal.
2. Have an annual wellness visit and complete the corresponding form found in the member portal.

Members must be 18 years or older to participate. For more information about this program, log into the member portal.

Member Wellness
Network Health’s member wellness program offers one-on-one health coaching, group support and individual learning resources to help you achieve and maintain a healthy lifestyle.

Care Management 
Whether you’re dealing with a short-term illness or a more complex condition, personalized care management supports you in getting the health care you need.

Condition Management
Condition management is available to members living with asthma, chronic obstructive pulmonary disease (COPD), diabetes, heart disease, stroke or heart failure. Our skilled team of registered nurses engages members in one-on-one health coaching about their ongoing care, provides educational resources and helps build connections to community programs.
