Everyone has different insurance needs. That’s why Network Health offers a variety of plans to ensure you have the quality coverage you need at an affordable cost.

All Network Health’s Prestige plans are available on the health insurance exchange, which is also known as the Marketplace. These plans are HMO (Health Maintenance Organization) which means you’ll have to use our high-quality network of doctors, hospitals, clinics and specialists for services to be covered. This includes labs and services your doctor may refer, so you’ll want to ensure all providers you use are in-network.

As you review our plans, remember that you may qualify for a subsidy from the government. If you have questions, call us at 844-635-1322, Monday-Friday from 8 a.m. to 5 p.m.

A subsidy is financial help provided by the government to help people afford health insurance. There are two types of subsidies – tax credits and cost-sharing reductions. A tax credit helps make your monthly premium payment lower. A cost-sharing reduction lowers the amount you pay for health insurance deductibles, copayments and coinsurance. If you qualify for a cost-sharing reduction, you must obtain coverage through Marketplace to receive the benefit.
HSA stands for Health Savings Account. This savings account lets you set aside pre-tax money to pay for qualified medical expenses. Qualified medical expenses include deductibles, copayments, coinsurance, and many other medical expenses. Since HSA accounts are associated with high-deductible plans, the pre-tax dollars can offset some of your expenses.
If you live in Calumet, Outagamie or Winnebago county in northeast (NE) Wisconsin, you'll have access to our northeast Wisconsin provider network. Use our NE Find a Doctor and Find a Facility search to easily find providers in this network.  If you live in Washington, Ozaukee, Milwaukee, Waukesha or Racine counties in southeast Wisconsin, you'll have access to our southeast Wisconsin provider network. Use our SE Find a Doctor and Find a Facility search to easily find providers in this network. 

Prestige Bronze Essential

This plan offers a low monthly premium and great coverage. Your first three primary care doctor visits are $0 and then just $30 per visit after that. The coinsurance is 50 percent.

Prestige Bronze 20 HDHP

In addition to a low monthly premium and quality coverage, this plan offers 20 percent coinsurance. This plan, as displayed here, may be combined with a health savings account (HSA).

  • $6,750 Individual/ $13,500 Family Maximum Out of Pocket
  • Prescription Drug Coverage

Prestige Bronze 0

New for 2019. You’ll pay $0 for most medical services once you meet your deductible. There are separate copays and coinsurance for prescription drugs.

Prestige Bronze 50 HDHP

This plan has the lowest deductible of all the Bronze plans. This plan, as displayed here, may be combined with a health savings account (HSA).

Prestige Silver Essential

This plan has a reasonable deductible and low copayments for common medical services. Your first three primary care doctor visits are $0 and then just $25 per visit after that.

Prestige Silver 20 HDHP

This plan offers a deductible and coinsurance midway between the Bronze and Gold HDHP plans. This plan is HSA qualified when a government cost-sharing subsidy is not applied.

Prestige Gold Essential

This plan offers a low deductible and low copayments for common medical services. Your first three primary care doctor visits are $0 and then just $20 per visit after that.

Prestige Gold 50

This plan offers broad coverage with the lowest deductible of all our plans. It also has the lowest out-of-pocket maximum.

Prestige Gold 0 HDHP

New for 2019. This is Network Health’s first gold-level HSA qualified plan. After you meet your deductible, you’ll pay $0 for most medical services. There are separate copays and coinsurance for prescription drugs. This plan, as displayed here, may be combined with a health savings account (HSA).
