Care management provides personalized coordination of care, access to care, identification of benefits and resources, and condition monitoring to help you get the health care you need, whether you’re dealing with a short-term illness or a more complex diagnosis.
Care management begins with a one-on-one conversation with a care manager. If you choose to participate, you will work directly with a care manager to manage your health condition with the ability to opt out at any time by contacting the member experience number on your ID card. Care management is available at no cost to members.

Our condition management team empowers you to take charge of your chronic condition and be the healthiest you can be. Our team of registered nurses engages members in one-on-one health coaching about their ongoing care, provides educational resources and helps build connections to community programs. Condition management is available at no cost to members, and enrollment in the program is voluntary. Individuals who may benefit from condition management services include those who have any of the following conditions.

Breathe at Ease is Network Heath’s proactive condition management program designed to help members and participants avoid flare-ups, control symptoms and potentially improve their lung conditions. It’s intended to supplement a physician’s care of chronic bronchitis, emphysema and chronic obstructive pulmonary disease (COPD).
The program is conducted by a registered respiratory therapist (RRT) or certified respiratopry therapist (CRT) from an outpatient pulmonary department. A telehealth option is also available. Once enrolled, you’ll have a one-on-one meeting with a Breathe At Ease respiratory therapist and receive an assessment, discuss your health history and develop and action plan.

Our Prevent T2 Lifestyle Change Program is a yearlong program designed for people with prediabetes who want to lower their risk of developing type 2 diabetes. Designed to help you make small changes that make a big difference to your health, this proven program is facilitated by Network Health lifestyle coaches and is part of the National Diabetes Prevention Program.
Prevent T2 empowers individuals by focusing on the following lifestyle and risk-mitigation strategies.
To be eligible for Prevent T2, you must have a fasting glucose of 100 – 125 mg/dL and a Body Mass Index (BMI) greater than 25.
