Care Management

Whether you’re dealing with a short-term illness or a more complex condition, personalized care management supports you in getting the health care you need, at no additional cost. Care management begins with a one-on-one conversation with a care manager. If you choose to participate in an ongoing program, you will work directly with a care manager to manage your health condition with the ability to opt out at any time.

What to Expect from Care Management

Your care manager helps to monitor and support your health care by:
  • Providing assistance in completing health risk assessments
  • Working with you to develop goals and an individualized care plan
  • Helping you get services you need to manage your care and put you in control of your health
  • Working with your health care providers to coordinate care and communicate your care plan
  • Reviewing your medications and treatment plan
  • Informing you about the signs that your health condition is worsening, including when you should contact your personal doctor
  • Including you in your health care decisions
  • Helping you identify and develop your own community support system
  • Sharing various community resources that may be helpful
