Coverage for Your Smile

At Network Health, we know how important it is to protect your smile. Preventive dental care can help keep your teeth and gums healthy. Your Network Health plan offers one exam, one cleaning and one bite-wing x-ray at a $0 copayment to all covered members of your plan. Please review your Summary of Benefits and Coverage (SBC) for more details.

Oral exam

You are covered for a comprehensive or periodic oral exam. Your dental benefit includes one oral exam per year for each covered member of your plan, including children.

Annual Cleaning

Your plan covers adult and pediatric cleaning, once annually. This cleaning will remove plaque and tarter from your teeth. By removing the plaque that causes tooth decay and gum infections, your dentist can help you achieve better oral health.


You are also covered for one bitewing x-ray per year. Bitewing x-rays take an image of the upper teeth, lower teeth, roots and supporting bone of the teeth. These x-rays are used to detect decay between teeth.

Beginning January 1, 2020, you can contact Argus Dental at 844-296-1850. If you have questions before then, please contact Network Health customer service.

To use this benefit, simply show your Network Health member ID card at an in-network provider. Visit Argus Dental's website to Find a Dental Provider. Under the Plan dropdown, select Network Health, complete the other information and click Search.

Protect Your Vision

An annual eye exam helps you ensure your health and well-being by spotting early warning signs for a variety of health conditions like diabetes and blood pressure.

With EyeMed through Network Health, all covered members of your plan can have one annual eye exam for a $0 copayment, at a participating EyeMed provider. Please refer to your Summary of Benefits and Coverage (SBC) for more details. If you have questions about your benefit, beginning January 1, 2020, you can contact EyeMed at 866-939-3633 (TTY 711) Monday-Saturday from 7:30 a.m. - 11 p.m. and Sunday from 11 a.m. - 8 p.m. EST. Until January 1, 2020, please contact Network Health's customer service team.

Fitness Benefit (for Essential plans only)

Network Health has teamed up with the Active&Fit® program to offer a fitness benefit at no additional cost to all covered members of an Essential Prestige Plan. This benefit offers two options to improve or maintain your physical fitness.

1. Fitness Center Program - Provides access to 11,000+ participating fitness centers, including conventional gyms and studio classes such as yoga, cycling and more. With this option, you can join one gym or multiple gyms, whichever is most convenient for you.


2. Home Fitness Program - Provides members with their choice of up to two home fitness kits per benefit year. Each kit is unique and focuses on a specific style of fitness. Depending on the kit(s) you choose, it may include such useful items as DVDs, guides, a FitBit activity tracker, and/or other items to help you get fit from the comfort of your home.

Regardless of the option you choose, all Active&Fit members have access to the Active&Fit website, which offers these features to help you make the most of your benefit.

  • The Active&Fit ConnectedTM tool allows you to track your exercise and activity from a wearable device—this is also available through Active&Fit's ASHConnectTM mobile app*
